Where can I get a patch like this?

I want to make myself a colorblock sweater that I saw someone make for a different team and was hoping to add an embroidered patch to it. Someone in a fb page I'm part of posted that they make headbands and hats and stuff. Multiple people asked where she got the patches and she just ignored those comments while responding to others. I'm hoping someone here can help, so does anyone know where I can get a good quality patch?
